The Global Dental Access Days Foundation (GDAD) is led by a passionate and experienced team, including several founding members of the Central Arizona Dental Society Foundation (CADSF). Many of these visionary leaders played a pivotal role in launching the Arizona Dental Mission of Mercy (AZMOM) events in 2012, which provided free dental care to thousands of patients over the past decade, and our new board members bring a wealth of passion, energy and enthusiasm for the charitable clinics we offer to those who have no other access dental care they often desperately need.
Dr. Pat Rabot
DMD, Scottsdale Oral & Maxillofacial Surgery (President)
Bio
Dr. Rabot is a Board Certified Oral and Maxillofacial Surgeon (American Board of OMS), starting a private practice in 1993, and on staff at all HonorHealth Scottsdale’s three (3) campuses (Osborn, Shea and Thompson Peak). He is a founder and past president of the Central Arizona Dental Foundation (CADSF) and its free dental clinic known as Arizona Dental Mission of Mercy (AZMOM). Recently inducted into the Arizona Dental Association Hall of Fame, Dr. Rabot has served many professional societies including serving as Specialist Consultant and Anesthesia Office Evaluator for the Arizona Board of Dental Examiners; President of the Phoenix Society of Oral and Maxillofacial Surgeons, and; Chairman of the Arizona State Dental Association’s Council on Communications. He also served as a Hospital Corpsman for the U.S. Navy for five (5) years. Dr. Rabot completed his undergraduate degree at the University of California, San Diego, CA; his DMD degree at Washington University in St. Louis, MO, and his Oral and Maxillofacial Surgery Residency at the University of California, San Francisco, CA. He completed a Fellowship at San Francisco General Hospital and was an appointed, Associate Clinical Professor at UCSF in the department of Oral and Maxillofacial Surgery.
Violet Michael
EA, MST, Kong and Associates (Treasurer)
Bio
Ms. Michael has over 21 years of accounting experience including financial analysis and planning, financial statements and reporting, accounting policies and procedure, personal and corporate taxes, sales and use taxes, business and personal property taxes, and business process improvement. Violet has helped multi-million dollar clients get their business in compliance in all areas of tax and internal controls, having formed strong relationships with some of the firm’s top clients. Violet served as Treasurer on the board of the Central Arizona Dental Society Foundation. She also volunteers for the Osborn School District, helping to raise funds and supplies to help children who live in this under-served area. Formerly a board member of The Women’s Enterprise Foundation, Ms. Michael continues to support fundraising efforts geared toward financing scholarships and grants for emerging women business owners. Ms. Michael earned a bachelor’s degree in accounting from New Mexico State University, where she minored in Spanish, and received a master’s degree in taxation from Golden Gate University.
Lisa Campos

Ms. Campos brings financial expertise with a background in business banking and lending. Currently with First Citizens Bank, Ms. Campos held prior roles at Bank of America.
Dr. Dale R. Hallberg
DDS, Hallberg Family Dentistry Retired (Director)
Bio
Dr. Hallberg is a lifelong advocate for community dentistry and access to care, with more than 40 years of experience serving patients in northern Arizona.
A graduate of Northwestern University Dental School (Class of 1985), Dr. Hallberg owned and operated Hallberg Family Dentistry in Flagstaff until his retirement in 2025. Throughout his career, he has been deeply involved in organized dentistry and community service, holding lifetime memberships with the American Dental Association (ADA), Arizona Dental Association (AZDA), and Northern Arizona Dental Society (NADS).
He has served as Board President of the Poore Free Medical Clinic / Sid Davis Memorial Dental Clinic, where he continues to volunteer his time providing care for underserved populations. Dr. Hallberg has also been a dedicated volunteer for every Arizona Dental Mission of Mercy (AZMOM) event in Phoenix and served as Director of the Flagstaff event. In addition, he spent 11 years on the Delta Dental of Arizona Board of Directors, where he chaired multiple committees and helped guide statewide initiatives to expand access to dental care.
A member of the Northern Arizona Network for Dental Excellence study club, Dr. Hallberg remains active in mentoring and supporting professional collaboration among dental colleagues. He also volunteers with the New Mexico Dental Association Foundation Denture Clinic, reflecting his ongoing commitment to restoring health, dignity, and smiles for patients across the Southwest.
Dr. Hallberg and his wife, Terri, reside in Flagstaff and are the proud parents of two sons—both practicing dentists.
Dr. Brett Dameron
DDS, Phoenix Cosmetic Dental Care (Director)
Bio
Dr. Dameron is a past President of the Central Arizona Dental Society Foundation (CADSF) and its free dental clinic known as Arizona Dental Mission of Mercy (AZMOM). He has been a Phoenix Magazine top dentist since 2012 and is a member of numerous professional associations including the American Dental Association; Arizona State Dental (AZDA) Association; American Academy of Cosmetic Dentistry; Academy of General Dentistry; Dental Organization for Conscious Sedation; American College of Oral Implantologists; American Academy of Facial Esthetics, and as a diplomate in the American Sleep and Breathing Academy.
Kristi Deela
CDA, CPFDA, CRFDA, MHA (Director)
Bio
Ms. Deela has 20 years of experience in the dental field including work as a chairside dental assistant, lead dental assistant, front office assistant, back-office manager, and obstructive sleep apnea assistant as well as dental practice analysis and OSHA training. After 10 years of clinical experience, Ms. Deela pursued a focus on dental education, while still involved in private practice to fuel her knowledge and passion for patient care. Currently serving as the Program Director for Phoenix College’s CODA Accredited Dental Assisting Program, Ms. Deela enjoys shaping the next generation of dental assistants through education and mentorship. She volunteered every year for 10 years with Arizona Dental Mission of Mercy (AZMOM), including 8 years serving as Sterilization Lead. Ms. Deela also contributes to the community by volunteering at local schools as well as supporting and organizing various dental initiatives. Ms. Deela also volunteers with organizations such as American Dental Education Association (ADEA), Dental Assisting National Board (DANB), Arizona Advocates for Dental Assisting, mentoring future dental professionals and assisting with special projects
Karl Frye
Principal Owner and Founder -Frye Practice Sales & Healthcare Real Estate (Director)
Bio
Mr. Frye started Frye Practice Sales & Healthcare Real Estate after over a decade in Commercial Banking where he served a client portfolio of doctors, helping them obtain the credit needed to start, grow, or sell their practices. After identifying a need among Dentists, Veterinarians, and other Healthcare Practice Owners for a fiduciary service based on the ever-changing healthcare market nationwide, Mr. Frye started Frye Practice Sales in 2015. Since then, Mr. Frye has helped hundreds of Dentists, Veterinarians and other Healthcare Practice Owners nationwide realize their dreams for ownership, retirement, or joining a Group or DSO by providing exclusive representation on both their business and real estate needs. Mr. Frye offers credit acumen that rarely exists in the field, setting his services apart from other brokers, stemming from his 12+ years of experience with national and regional banks where he financed acquisitions, sales, refinances, and buildings for healthcare professionals.
Dr. Jayson Kirchhofer
DMD, MD North Scottsdale Oral Surgery (Director)
Bio
Jay is a Board Certified Oral and Maxillofacial Surgeon who has been practicing in “The Valley” since 2021. Jay is currently on staff at multiple hospitals and provides call services at two Level 1 Trauma Centers treating a vast array of pediatric and adult head and neck soft tissue injuries, facial fractures, and emergent dental conditions. Jay also assists in the training of Oral & Maxillofacial Residents at Banner University Medical Center as a Clinical Assistant Professor for the University of Arizona College of Medicine. Jay completed dental training through the DMD Dental program at Oregon Health & Sciences University where he engaged in community programs including the “Mission of Mercy” in Portland, OR and the “Compassion Clinic” in Tigard, OR. Jay completed Oral and Maxillofacial Surgery Residency training, and obtained his MD medical degree, at the University of Florida. While in Residency, Jay served the Jacksonville, FL Misson of Mercy, and a Medical Mission Trip to the Dominican Republic with the University of Florida, College of Medicine. Jay served as a co-lead for the Oral Surgery section of the 2023 Mission of Mercy in Phoenix, AZ, and is also currently a Director on the board of the Central Arizona Dental Society.
Jeremy Jay Smith
Global Denture Alliance (Director)
Bio
Mr. Smith served as Founder and Chief Executive Officer of Global Denture Alliance from 2017 – 2024 when the organization was renamed and repurposed to expand beyond providing dental prosthetics to include comprehensive dental care for underserved communities. Mr. Smith is a third generation Dental Technician/Denturist with a history of working in the Dental & Health Care industry, with Nonprofit Organizations, and providing removable prosthetic training, customer service, strategic planning, and public speaking services. Mr. Smith completed training in Dental Technologies from Milwaukee Area Technical College and Las Vegas Institute of Advanced Dentistry
Dr. Emily Tarr
DMD, Market Street Dental Studio (Director)
Bio
Dr. Emily Tarr began her dental career as a chairside dental assistant, gaining six years of hands-on experience before entering dental school in 2021. Her passion for expanding access to oral health care was sparked during a 2013 outreach trip to Honduras, where she helped establish pop-up dental clinics in communities around Tela. By 2019, her leadership had grown into the role of Director of Dental Operations for One World Brigades in Oregon, supporting international service initiatives.
While attending Midwestern University College of Dental Medicine Arizona, Dr. Tarr continued her commitment to community service. As president of her school’s chapter of the American Student Dental Association, she organized an Oral Cancer Walk, an Oral Hygiene Kit Drive for Maggie’s Place, and District 10 conferences in San Diego and Seattle.
Dr. Tarr has actively contributed to events like Give Kids a Smile and Arizona Mission of Mercy, demonstrating her drive to improve access to care, promote oral health education, and serve vulnerable populations. She remains equally dedicated to advancing patient care, advocating for the dental profession, and contributing to research on the oral-systemic health connection. Her mission is built on creating meaningful, lasting change in the field of dentistry.
Phil Whitaker
Legal Counsel, Frazer Ryan Goldberg & Arnold LLP (Director)
Bio
Mr. Whitaker has broad legal expertise, emphasizing trial and arbitration work. His experience includes proceedings involving a wide spectrum of business-related issues, including disputes arising from the sale of businesses to the sale of goods, and commercial tort cases, such as fraud, embezzlement, and breach of fiduciary duty. He has won jury trials representing plaintiffs and defendants, and he handles appeals in both the state and federal courts. Phil also advises business clients in contract and lease matters, entity selection and formation, and the drafting of entity governance documents, including shareholder agreements, operating agreements, employment, and independent contractor agreements. Mr. Whitaker served on the CADSF / AZMOM board for many years. He earned his J.D. from Hastings College of the Law, University of California, 1987, and his A.B. in History from University of California, Riverside, magna cum laude, 1984
Tyler Banks

Tyler Banks serves as the General Manager for Patterson Dental in Arizona, where he leads a dedicated team committed to helping dental practices thrive. With nearly 11 years at Patterson, Tyler has developed extensive experience in supporting clinicians through technology innovation, dependable service, and collaborative partnership. His leadership approach centers on listening first, understanding each practice’s unique goals, and tailoring solutions that enhance both patient care and business performance.
Tyler holds a master’s degree in Business Management and Leadership from Arizona State University and believes that success in dentistry is always a team effort. He is proud to partner with dental professionals across Arizona to build healthier practices, stronger communities, and a brighter future for oral healthcare.
Kevin Conroy

Bio
Mr. Conroy provided professional oversight of the Central Arizona Dental Society Foundation (CADSF) and its Arizona Dental Mission of Mercy (AzMOM) free dental clinics in a pro-bono capacity for over ten years, serving as Executive Director. Also serving as the Operations Director at Phoenix Endodontic Group. Prior to joining his wife, Dr. Jacqueline Allen, in 2005 as OM for their Endodontic dental offices, Mr. Conroy worked for twenty-two years as a Senior Vice President in the equipment leasing field for both AT&T Capital Corporation and Wells Fargo Financial Leasing. Mr. Conroy had responsibility for 32 direct reports and P&L responsibility for a sales budget in excess of $300 million. He utilized this financial expertise in forming CADSF to operate in a highly efficient manner through the engagement of volunteers (including dentists, other dental professionals and community volunteers) and utilization of independent contractors who worked remotely, thereby minimizing the costs of paid staffing and eliminating the need for office space and corresponding overhead costs. For years, Mr. Conroy has traveled with Dr. Allen to many other states to take part in Missions of Mercy as a general volunteer and it was there that he developed a vision for the non-clinical aspects of the event in Arizona. In 2017, Kevin was elected to the Board of Directors of the America’s Dentists Care Foundation in Wichita, KS, and in 2020 served as its Board President He received a Bachelor of Science degree from James Madison University in 1982.
Emily Wu
Executive Director
Bio
Emily has 25 years of experience serving the nonprofit sector as a professional grant fundraising consultant, executive director, board member, and volunteer. She serves clients throughout the country, representing a wide range of health and human service, climate change, justice, technology, civic engagement, and other initiatives. Emily consistently achieves uncommon results which she attributes to her unique combination of experience serving the nonprofit sector, and prior ten-year banking/finance career. Having a strong passion for serving the “greater good”, Emily left banking to contribute her skills to nonprofit clients striving to address important issues. After completing formal credit training with a major regional bank, Emily managed credit underwriting for several lending divisions, bank-wide regulatory compliance projects, credit training for MBA graduates, and loan portfolios of up to $150 million, rising to the Vice President Corporate Loan Officer level. Her wide range of credit experience includes major corporate, real estate, technology, franchise, nonprofit, hospital, and other client initiatives. Emily authored credit underwriting policies and white papers for Asset Based, Non-Profit & Health Care, and Utilities lending divisions, as well as white papers supporting the launch of Franchise Finance and Assisted Living lending divisions. She has also instructed courses in Intermediate Accounting, Cash Flow analysis, and Writing Effective Credit Analysis. Emily earned a B.A. in Business Administration at Boston College, Chestnut Hill, MA, 1989.
